Background: It is established that a low body mass index (BMI) correlates with a diminished home discharge rate and a decline in activities of daily living (ADL) capacity among elderly stroke patients. Nevertheless, there exists a paucity of knowledge regarding strategies to mitigate BMI reduction during the acute phase. This investigation seeks to elucidate the impact of rehabilitation dose, as determined by both physical and occupational therapy, on BMI alterations, positing that a heightened rehabilitation dose could thwart BMI decline. Methods: This retrospective, observational study was conducted in the stroke unit of a university hospital. Enrollees comprised individuals aged ≥65 years, hospitalized for stroke, and subsequently relocated to rehabilitation facilities between January 2019 and November 2020. The percentage change in BMI (%ΔBMI) was calculated based on BMI values at admission and discharge. Multivariate multiple regression analysis was employed to ascertain the influence of rehabilitation dose on %ΔBMI. Results: A total of 187 patients were included in the analysis, of whom 94% experienced a reduction in BMI during acute hospitalization. Following adjustment for sociodemographic and clinical factors, multivariable analyzes revealed a positive association between rehabilitation dose and %ΔBMI (ß = 0.338, p < 0.001). Conclusion: The findings of this study suggest that, in the context of acute stroke treatment, an augmented rehabilitation dose is associated with a diminished decrease in BMI.

PURPOSE: Potentially inappropriate medications (PIMs) are a major concern in geriatric care. Osteoporotic vertebral compression fractures (OVCFs) are more common among the elderly, who are more likely to receive long-term analgesics for pain and additional medications for complications, but no studies have examined the effects of PIMs on OVCFs. The purpose of our study was to clarify the association between PIMs and activities of daily living (ADL) among patients with OVCFs. METHODS: The subjects were 170 patients aged 65 years or older with OVCFs treated conservatively who underwent rehabilitation between October 2014 and August 2019. Patients' clinical information, including age, sex, body mass index, total number of drugs used for treatment at admission, number and type of PIMs used at admission, bone mineral density, use of drugs for osteoporosis, fracture type, comorbidity severity, nutritional status, Barthel Index (BI), presence and extent of lower back pain, and length of hospital stay were examined retrospectively. The possible association between these clinical factors and the BI and use of PIMs was assessed. RESULTS: Ninety-seven patients (57.1%) were prescribed PIMs at admission. Mean BI at discharge was significantly lower among patients receiving PIMS (70 points vs. 83 points; p < 0.001). Multivariate analysis for BI gain after propensity score matching showed that the number of PIMs and BI at admission affected the BI gain. CONCLUSION: The use of PIMs hindered the improvement in ADL. Our results demonstrate the importance of appropriate drug control for patients with OVCFs.

RESUMO

A liquid-core liquid-cladding optical waveguide based on thermal gradients across laminar flow was built with the laminar flow of water in a stainless capillary tube placed in a heat source. Its characteristics were studied with both experiments and a computational fluid dynamics simulation, firstly showing that it had the nature of a graded index optical fiber.

A stable two-phase sheath flow using tetrahydrofuran (THF) for an inner flow and water for an outer flow was formed in a glass capillary, and worked as a stable liquid-core/liquid-cladding optical waveguide (THF/water LLW). Although THF and water were miscible with any ratio, the length of the stable THF/water LLW at 0.9 - 2.1 cm s-1 reached at least 150 mm. The THF/water LLW was applied to the observation of extraction behavior of solvatochromic fluorescence dye, 1-anilino-8-naphtalene sulfonate (ANS), through the THF/water interface. ANS was added to the water phase (clad solution) and its fluorescence, which was excited with the guided light (355 nm) through the LLW, was observed by changing the position of the detector. While the ANS stayed in the region of 70% THF to the end of the LLW without the addition of cationic surfactant, hexadecyltrimethylammonium ion (CTA+) at pH 3 and 11, the ion-pair of ANS and CTA+ was extracted into the higher concentration region of THF with the addition of CTA+ at pH 11.

A stable liquid/liquid optical waveguide (LLW) was formed using a sheath flow, where a 15% sodium chloride (NaCl) solution functioned as the core solution and water functioned as the cladding solution (15% NaCl/water LLW). The LLW was at least 200 mm in length. The concentration distributions of the liquid core and liquid cladding solutions in the LLW system were predicted by computational fluid dynamics (CFD) to validate the characteristics of the waveguide. The broadening of the region of the fluorescence of Rhodamine B excited by the guided light and the increase in the critical angle of the guided light with the increase in the contact time of the core and the cladding solutions were well explained by CFD calculations. However, no substantial leakage of the guided light was observed despite the considerably large change in the refractive index profile of the LLW; thus, a narrower and longer waveguide was realized.
